Health Requirements and FY24 Immunization Change Letter

The attached document should help you in determining if your child needs any health requirements met prior to the start of school.  Due to the difficulty in obtaining an appointment, it is possible for your child to start school with proof of an appointment for required physicals, exams or immunizations. Forms are available at our District Office or under the Parents & Community ~ School Documents.  You may also call Sue Cater, our School Nurse, at 815-872-1951.  Please leave a message if she is not available and she will return your call.
